About ConnectCare

One of the greatest challenges faced by employers today is managing the cost of providing quality health care to their employees. In 1993 ConnectCare was formed as a solution to that challenge.

ConnectCare Organizational Overview

ConnectCare is a joint venture between Physician Associates of Michigan P.C. (PAM) and MyMichigan Health. PAM is a physician organization which represents physicians throughout the area. Of those physicians about 30% are primary care practitioners and 70% are specialists. MyMichigan Health is a non-profit health system, headquartered in Midland, Michigan, affiliated with Michigan Medicine, the health care division of the University of Michigan. Recently named one of the nation's 15 Top Health Systems by Truven Health Analytics, MyMichigan Health covers a 22-county region with medical centers in Midland, Alpena, Alma, Clare, Gladwin, Mt. Pleasant, West Branch and Sault. MyMichigan Health also offers home health care, urgent care and physician services.

Mission Statement

It is our mission to provide a full range of efficient managed health care services through an Integrated Delivery System that is focused on the needs of our mid-Michigan customers.

This may be accomplished through directly partnering with employers, managed care organizations, and further development of ConnectCare.

Network

The ConnectCare PPO providers access to a large network of qualified practitioners; the Medical Centers in Midland, Alpena, Clare, Gladwin, Alma, Mt. Pleasant, Sault and West Branch, their affiliated facilities and numerous hospitals located throughout Michigan.

For care needs beyond what is locally available, ConnectCare, through an agreement with Cofinity or BCBSM, is able to present a nationwide network providing access to over 1 million healthcare professional service locations, 5,000 hospitals and over 90,000 ancillary facilities.

We are also pleased to pass 100% of our provider discounts directly to the employer unlike other large ASO plan Administrators.
